What are the hardest Part C Star Measures?   As part of the 2025 Star Ratings release this year, CMS also posted a couple tables containing the 2022-2025 Average Star Rating by Measure.[1] Two of the hardest measures, defined by the lowest average Star score, are C08 (Osteoporosis Management in Women who had a Fracture) and C12 (Reducing the Risk of Falling).
