Urging Congress to include prevention in how states use behavioral health grants is a no brainer! Support Kelly Davis, Congressmembers August Pfluger and Kathy Castor, and Mental Health America in this fight for youth #healthequity. Why do we wait for kids to hurt themselves, overdose, or hear voices when by then potential lifelong effects of trauma on their brains (not to mention schooling, friendship, siblings/parents, and others) have set in? In some ways this is the legacy of both stigma - acting like those more sick aren't human beings so let's ignore prevention of THEIR illnesses - and of a behavioral health industrial complex in the US that's been largely centered around pharmaceuticals and government/PE-consolidated psychiatric facilities. We pay for pediatrics to prevent broken lives. Public health to prevent epidemics. Why not behavioral health prevention services to prevent horrible diseases clearly impacting all of our children and communities? #mentalhealthishealth. Let's get kids there by not acting like mental illness or addiction are forgone conclusions.

Yesterday CommonWealth Beacon published my perspective on the evolving Steward Health Care crisis in Massachusetts. As our state leaders contend with the many uncertainties ahead and influence the future of these critical access points, #healthequity and health equity leaders must be at the center of the discussion. Amidst the turmoil, we have the opportunity to impact three important determinants central to health: 1. Creating and maintaining opportunities for health - economic stability 2. This 20-year follow up report reveals that while rates of uninsured have declined, we have not yet reduced health disparities in the United States. Until we recognize the multi-layered impact of community assets, politics/legislation/health policy, systems of care, healthcare provider and individual factors and accordingly implement multi-level approaches with measurable accountability across each layer as well as make visible the invisible, change will continue to move slowly.

For nearly 30 years, the Black Women's Health Study has been a groundbreaking pillar in understanding the unique health challenges faced by Black women. This incredible research: “Racism, Sexism, and the Crisis of Black Women’s Health”, led by Boston University, has uncovered startling disparities, including higher rates of hypertension, diabetes, and certain types of cancer. One of the study's most impactful findings is the connection between racism and health outcomes. Experiencing discrimination has been linked to increased stress, which can contribute to chronic health conditions. It's time to dismantle systemic barriers that impact the health and well-being of Black women. Let's come together as a community to support Black women's health. “The Maryland Infertility Mandates: Firsts Across the Board”- Good Policy Can Create Equity I hope there is a good appreciation of how powerfully policies at the state level influence healthcare and impact equity. In support of that hope, I co-authored this article with my colleague and friend Dr Eli Adashi, the former Dean of Medicine and Biological Science at Brown University and former Chair of OB-Gyn at the University of Maryland School of Medicine. The article describes how state legislators, influenced by grassroots supporters, took serial steps to make IVF broadly available and affordable in Maryland. The article also shows that when the "ice jam" is broken on hesitation to introduce good policies in one state, many other states find a clear channel to follow. Maryland leans toward progressive policies but even within that culture the state still must weigh economic impacts and other consequences of the laws they enact. Making laws is never simple, easy or a “slam dunk” process. The article recounts the way in which successions of lawmakers and Governors (from both parties) have addressed the equity impacts of insurance coverage policy for IVF in a thoughtful manner, sometimes taking several years to complete the task. And now, as there is a change of leadership at the national level, there will be speculation and uncertainty regarding national healthcare policy.  However, states will continue to have the authority and the responsibility to make policies that impact the health of all those within their borders. As many states found ways to provide insurance coverage for IVF that focused on human rights, so might they find a path though the ice to create healthcare access for all. Which state will break that ice and lead the way? In the closing line of the article we noted that “Remove all nonmedical biases against IVF, and a presidential signature should all but be assured”. This line was written even before the President elect declared himself “The Father of IVF” and perhaps provides hope on the federal level. Yours in health Howard

Thank you Coral Health Advisors (@coral_healthadv) for spotlighting the Commonwealth Fund's report comparing / contrasting hospital global budgeting experiences across US, Canada, Netherlands, New Zealand and Norway. This concept gets talked about often by healthcare leaders, as if its the holy grail. However, this report affirms my observations: - 'Moving the cheese($$) levers' will always keep the healthcare systems attentive. After all, no one wants to go out of business and leave a community without important services. - As the Commonwealth's report states "When you’ve seen one nation’s hospital global budgeting scheme, you’ve seen one nation’s hospital global budgeting scheme." Decades of trial and error of creative financial funding arrangements have not yet proven success much less an ability to be replicated. Its admirable and aspirational to motivate our healthcare institutions to evolve in effectiveness and efficiency. No one believes it more than me - its what I love to do in my business, Advance Healthcare. However, I would be encouraged if we, as a country, paid an equal amount of energy to test incentive programs directly with the people, that have an objective of shifting our population's HEALTH trajectory. Many may say that cost is a proxy for health, but I believe it disorients us away from the root opportunity. If we repositioned our mission to improving the 'health trajectory' it would initiate efforts beyond the soul focus of healthcare systems and $10 off per month for a gym memberships. It would expand to industries throughout our nation that keep us as healthy as possible like: - Food companies, - Behavioral / mental services - Spiritual care environments and activities - Access to nature and exercise, and - Elderly/care giver support to name a few. This is what sits upstream of the health system's costs. That would be aspirational. Indeed a turning point for women's health. I am not a political person, but this is something I can proudly support. Hot off the presses! It was great to work with Prajakta Adsul, MBBS, MPH, PhD, Rachel Shelton, April Oh, PhD, MPH, Nathalie Moise, MD, MS, FAHA, and Juliet Iwelunmor-Ezepue, Ph.D.! Implementation science focuses on enhancing the widespread uptake of evidence-based interventions into routine practice to improve population health. However, optimizing implementation science to promote health equity in domestic and global resource-limited settings requires considering historical and sociopolitical processes (e.g., colonization, structural racism) and centering in local sociocultural and indigenous cultures and values. This review weaves together principles of decolonization and antiracism to inform critical and reflexive perspectives on partnerships that incorporate a focus on implementation science, with the goal of making progress toward global health equity. From an implementation science perspective, wesynthesize examples of public health evidence-based interventions, strategies, and outcomes applied in global settings that are promising for health equity, alongside a critical examination of partnerships, context, and frameworks operationalized in these studies. We conclude with key future directions to optimize the application of implementation science with a justice orientation to promote global health equity. https://lnkd.in/egVSPYN9
