Students gained hands-on experience in healthcare, working with various departments to develop innovative solutions for health equity challenges. Thank to Rich Besser for sharing such a thoughtful reflection on the milestones achieved by the Robert Wood Johnson Foundation in 2024 and their steadfast commitment to health equity. It is truly inspiring to see leadership and the RWJF's unwavering dedication to making health a right, not a privilege, especially during a time when the principles of Diversity, Equity, and Inclusion are being challenged and re-examined. This work reminds us that transformative change requires both persistence and collaboration. I would like to offer some thoughts on next steps that might further advance the RWJF mission, particularly through the lens of complexity science, stress-energy dynamics, and bio-neurodiversity: 1. Expand the Lens on Health Equity through Complexity Models Health is an emergent property of complex systems, shaped not just by access to resources but by stress-energy dynamics and how well environments nurture diverse human needs. Exploring how these dynamics affect different bio-neurotypes can offer innovative ways to address health disparities and systemic inequities. Next Step: Invest in research and programs that integrate complexity science into health equity initiatives, highlighting how diverse neurotypes and adaptive strategies shape health outcomes. This approach can illuminate the role of stress, resilience, and environmental mismatch in health disparities. 2. Advocate for Ecosystem-Based Health Solutions Modern health systems often emphasize linear, individual-focused interventions, which can miss the interconnected nature of health determinants. A shift toward ecosystem-based health solutions that consider social, nutritional, psychological, and environmental factors is essential to creating sustainable health equity. Next Step: Foster partnerships with organizations developing ecosystem-based health frameworks. Encourage pilot programs that address systemic barriers while nurturing cooperation and diversity within communities. 3. Center Adaptive Diversity in Health Policies The RWJF’s efforts to increase representation in academic medicine and public policy are remarkable. Expanding these efforts to recognize and support bio-neurodiversity—how different cognitive and stress-response profiles impact health—can further dismantle systemic biases in healthcare and health policy. Next Step: Support training for healthcare providers and policymakers on the importance of adaptive diversity. This could include developing resources to better understand how neurodivergence and stress dynamics intersect with health inequities. Thank you again for your leadership, and I look forward to seeing the transformative impact of your efforts in 2025.

"That was fellow-level research, Destiny, congratulations!" Dr. Abigail M. Ramseyer, DO And that's a wrap! Over two years of research have led to this morning's opportunity to present at the Sparrow Health System's Grand Rounds! It was such an honour to be invited by Stephanie Fleming, Chair, Dept of Ob/Gyn Sparrow Hospital, following my presentation at the #HEALSexualHealthConference2024 at Michigan State University! My research on the overutilisation of emergency services is multifold; 1. Identifying patients seen in Michigan hospital systems (Urban, 100000+ populations) 2. Determine overutlisation status based on ICD-10 codes, provider discharge summary, and patient recommendations. Encourage survey participation. 3. Provide educational interventions (video, brochure, poster) and send out retake attempts to survey participants following a 30-day period. This research not only highlights significant levels of overutilisation in Michigan but also which education interventions and resources are most effective in developing informed decision-making from our patients, thus relieving the burden on our emergency staff and providers. Lown Institute's annual Fair Share Spending Report drew the American Hospital Association's ire as outlined in a statement yesterday. The AHA's defense of current practices around community benefits are not new, nor are the Lown Institute's reasoned rebuttals. The same day the report was issued citing the Cleveland Clinic with a $202 million "fair share deficit," an architectural rendering was released showing of the future Cleveland Clinic Global Peak Performance Center. With Cleveland Clinic's name front and center, it "will blend elite training with sports medicine, nutrition and other wellness services." The AHA's sharp criticism of the report and its methodology focuses on the value of Medicaid and Medicare services, research, and education. They slam the policy recommendations laid out in the report, which largely focus on more transparency and reporting requirements. So basically, "You're wrong about how much we contribute to our communities, but we're not actually going to be more transparent to prove it." Ultimately, the goal should be to ensure that the substantial tax benefits enjoyed by nonprofit hospitals are matched by meaningful, impactful investments in community health. Building a 200,000 sq. ft. elite athletic performance center doesn't seem consistent with this goal. The dialogue initiated by the Lown report and the AHA's response will likely continue, with no real policy changes in the near term. The 2025 report will probably make the same findings and Mr. Pollack will march out the same response. I do hope that I am proven wrong on these last points.

There are six words that outline Houston Methodist's vision for the second century: unparalleled safety, quality, service and innovation. You might be thinking — does the “and” really count? Absolutely, believes Houston Methodist President and CEO Dr. Marc Boom. In fact, it might count most of all. None of these are tradeoffs: There's no place for “or” in top-notch healthcare. "These aren't choices," he explained. "You don't sacrifice service for safety or vice versa." To Houston Methodist, “and” is more than a word: It’s a cultural norm foundational to the system’s sterling reputation, one further cemented by all seven of its hospitals landing in the top decile of the 2023 Vizient Quality and Accountability rankings. So, what does “better” look like when you're one of the best? "It's never about the awards — but we are unapologetically competitive," Dr. Boom said, citing a laundry list of the impressive rival healthcare institutions that line the Houston landscape. 🌟 Last month, I had the profound honor of leading a transformative lecture for the Kaiser Napa Solano Family Medicine Residency’s Health Policy Elective 🌱✨ on the history of racism in America—and how everyday people, armed with cultural tools of truth-telling and collective iterative action, creativity and collective power, have forever changed health policies and outcomes. We navigated complex and emotionally charged themes: 🔍 The Susan Moore Paradox: Exploring the cruel weight of Misogynoir and how health outcomes of Black women are a barometer of the practitioner biases that are embedded into our healthcare standards. This topic left us all with a sense of despair and a fierce determination to dismantle it. Attendees shared stories that these biases aren’t abstract—they’re lived, painful realities that healthcare still struggles with. 💔 🌍 Transformative Congruence (shoutout to Dr. Desean Lee 🫶🏿): This idea of aligning cultural tools with professional care inspired a surge of hope in the room. 💪🏿 Each participant saw how they could actively leverage cultural humility as a force for both inclusion and innovation. It was dope to see so many people embrace my twin brother's draw to emergency medicine and simultaneously embrace the call to action for meaningful, culturally rooted care.🌱 ⚖️ The Dr. Simkins Effect: Here, we felt the power of small nudges that bend the arc of history toward justice. 🌈⚖️ From subtle actions to grand gestures, our discussions revealed how these “micro-interventions” build momentum by sharing interactive and collective tactics toward lasting change. Some attendees spoke of their own “nudges,” realizing that every intentional act can lead to systemic shifts. ✨ This session was more than a lecture—it was a moment to sit with the tension between heartbreak 💔 and hope 🌈. These powerful themes stirred a shared commitment to make health equity not just a goal but a lifelong journey of learning, unlearning, and doing. It was also validating to feel raw emotions, share real stories, and draw so much emergent insight. I felt more than unemployed, my data-informed thoughts and stories brought an energy to this virtual space that overshadowed my employment status. It was such an honor to have shared this space with such dedicated change-makers. I am more than grateful to have seen the sparks of transformation light up in the room. 🔥

A open-source methodology for estimating the transition from undergraduate to medical education. 💭Why care? We’ve linked this data back to high school enrollment, allowing for evidence driven workforce intervention design. 2. A chronic disease outcomes-based methodology to estimate primary care physician demand across the U.S. ✨What is it? An open-source population health based approach to quantifying the gap between disease burden and provider access. 💭Why care? We propose a modular and iterative framework to estimate physician pipeline training and recruitment needs (potential targeted programs) to meet future primary care physician demands based on population level burden.
